HOW TO PROVIDE YOUR MAILING LIST
Printable Format

LIST CONTENT
Provide only First Name, Last Name, Address, Address2, City, State and Zip.
Place the full name in two fields; ensure city, state and zip are in separate fields. Avoid middle initials.
Do not include titles (Mr., Mrs., Dr., M.D., Ph.D., etc.).

LIST SELECTION
Limit names to the 48 contiguous states. Do not include Alaska, Hawaii, APOs or foreign countries.
Make sure all state abbreviations are correct.

FILE FORMAT
Provide list in comma-deliminated ASCII text format.
Fixed-width ASCII text format is also acceptable.
ACCEPTABLE MEDIA
